Introducing our newest product, EEG Info Clinical Session Note Pads, inspired by Sue Othmer's clinical forms that she uses at the EEG Institute.

These preformatted session note forms are for use in your practice as they are made to be easily accessible and save you both printing time and costs.

Each note pad comes with 40 sheets, each containing room for client name, session #, date, day, time and clinician name. There is a table for recording reward frequencies and electrode placements and plenty of room for making notes during the session. Two holes have been drilled into the top for clinicians who use end-tabbed divided folders for their client records.
